Admission Criteria

Admission to Senior Behavioral Health Unit is indicated for patients ages 55 and older who have a psychiatric diagnosis or symptoms and meet the severity of illness of at least two criteria below:

  • Suicide attempts/ ideation
  • Self-destructive behavior
  • Homicidal ideation
  • Assaultive behavior
  • Destructive to property
  • Medication noncompliance
  • Disorientation or memory impairment causing change in daily living activities
  • Psychiatric symptoms such as: hallucinations, delusions, panic reaction, anxiety, agitation, depression

Documents needed for the intake process:

1) Demographic information– Face sheet

2) Copy of DPOA/ Guardianship

3) Physician Order for admission

4) Progress notes containing information on referral need

5) Medication list

6) Insurance cards

7) Recent UA/LABs if available

Exclusion Criteria:

Patients with C-diff, MRSA, VRE and other communicable diseases will be assessed on an individual basis. Patients with primary substance abuse disorder and patients requiring dialysis.
